Health Data. Examples: medical conditions, health or exercise activity monitoring, medical insurance information, Medicaid enrollment status, care needs assessments. We may share this with service providers.

Internet/Electronic Activity. Examples: IP address, device/browser data, user session activity. Used for internal analytics.

Inferences Drawn from Other Personal Data Collected. Examples: profiles reflecting user attributes, behavior, preferences, or abilities/aptitudes. We may share this with service providers.

"Shared" refers to disclosures made solely to provide our services as requested and directed by the user. These include disclosures to service providers or contractors under written agreement, as well as to other third parties to deliver the requested services. These are not considered "sharing" or "selling" under the CCPA/CPRA.

Tender Home Care is not acting as a Business Associate; Personal Data that you provide through the Site (such as when you complete eligibility forms, browse our website, or submit contact information) is not PHI or subject to HIPAA. 6. SMS and Mobile Communications

Your mobile information will be used by us to communicate directly with you regarding Services you have requested.
